Motorway worker killed while collecting traffic cones

A traffic management company has been fined £100,000 after supervision and signage failures ended with the death of an employee on the M4 near Cardiff.

The incident occurred on 8 September 2006, while Sean Hale was working with a fellow trainee traffic management operative as part of a project to resurface the motorway.
