When to use this model paternity leave form

Use this model form to give employees the means to provide notice to take paternity leave for leave taken on or after 6 April 2026, where both of the following apply:

• employees only become entitled to paternity leave because the previous 26-week qualifying service requirement will no longer apply from 6 April 2026; and
• the expected week of childbirth is in the period from 5 April to 25 July 2026 inclusive.
