When to use this model paternity leave form
Use this model form to give employees the means to provide a period of leave notice to take paternity leave.
Where employees only qualify for paternity leave because the 26-week qualifying service requirement no longer applies from 6 April 2026, and the expected week of childbirth falls in the period from 5 April to 25 July 2026 inclusive, see our Form to provide notice to take paternity leave (for births where transitional arrangements apply).
