A model form to get a declaration from a joint adopter attending adoption appointments as to whether they are electing to take paid time off under s.57ZJ of the Employment Rights Act 1996 or unpaid time off under s.57ZL of the Employment Rights Act 1996. The right to time off to attend adoption appointments is available to adopters from 5 April 2015.
